A permit to operate from the Health Division is required by State
or Local Regulations for the following activities or businesses
in the Town of Lexington.

Food Related Activity 

• New Food Permit - Food Plan Review Application
• Renovation of an existing Food Establishment - Modified Food Plan Review Application
• Temporary Food Permit - Application
• Food Establishment Renewal - Application

Commercial / Industrial

• Recombinant DNA Research - Permit Application
• Recombinant DNA Research - Renewal Application of an Existing License
• Septic Waste Haulers - Permit Application
• Well Construction (All types) - Application
• Well Completion Report
• Well Destruction (Closure) Permit - Application
• Rubbish Hauler - Application

Commmunity Health

• Day Camp / Recreation Camp - Permit Application
• Tanning Establishment -  Permit Application
• Tanning Establishment - Renewal Application
• Tobacco Sales - Permit Application
• Pool Permit Renewal Application

Note: All permit applicants are required to complete a Worker's Comp Affadavit.  Completed Affadavits must be submitted with the permit application.  A Worker's Comp Affadavit is not required to obtain a non - licensed temporary food permit.
