IMPORTANT NOTICE REGARDING MOTOR VEHICLE ACCIDENT REPORTS

The Massachusetts Registry of Motor Vehicles has recently discontinued the distribution of motor vehicle accident report forms to local police departments. Blank accident reporting forms will no longer be available at local police stations including the Lexington Police Station. These forms are available for download and printing in the Forms/Documents section of this site or by clicking the link below.
